29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway:    a.  On or about October 5, 2015 on the north side of a new commercial structure, an employee installing stucco to the facing of the building was using a scaffold that did not have guardrails, exposing the employee to a 13 feet 4 inch fall hazard.    b. On or about October 5, 2015 on the northwest side of a new commercial structure, an employee working on a scaffold to load a forklift did not have guardrails, exposing the employee to a 25 feet fall hazard.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access:  a.  On or about October 5, 2015 on the north side of a new commercial structure, an employee working on a frame scaffold did not use a ladder to exit and access the scaffold,  exposing the employee to a 25 feet fall hazard.  b.  On or about October 5, 2015 on the north side of a new commercial structure, two employees working on a frame scaffold did not use a ladder to exit and access the scaffold,  exposing the employees to a 13 feet 4 inch fall hazard.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:    a.  On or about October 5, 2015 on the north side of a new commercial building, employees working on a five buck frame scaffold did not use toeboards on the edges,  exposing the employee to struck-by hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a.   On the northwest side of the structure, an employee applying stucco material to the gable of a commercial building was not protected from a 25 feet fall hazard by the use of a personal fall arrest system.
