29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): In the manual lathe area of the facility at 270 Ashland Avenue Southbridge, MA: On or about October 5, 2015, the point of operation of the Summit model 28X60 manual lathe was not guarded while a machinist applied Mobil MET 766 metal fluid with an acid brush to a part being machined while the chuck and part rotated at approximately two-hundred and fifty (250) rotations per minute (rpm).
