29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  Exit route(s) were not kept free and unobstructed. An exit route located adjacent to the loading dock in the rear of the facility was found to be partially obstructed by multiple wheeled bakery carts. Exit access were not at least 28" wide, as the bakery carts reduced the pathway to approximately 25" wide, on or about October 5, 2015. Employees travelling down this exit path could contact carts, either inhibiting their escape, or could contact carts that would move into the part of travel and obstruct employees from exiting via this route.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Two vertical stand mixers located in the bakery production area were not provided with guarding to cover the rotating parts of the machine that mix the dough, exposing employees to point of operation hazards which could result in broken bone or amputation injuries, on or about October 5, 2015.   a. The Thunderbird vertical mixer was not provided with a cover guard to protect employees from the rotating parts that mix the dough, on or about October 5, 2015.  b. The Technopast vertical mixer was not provided with a cover guard to protect employees from the rotating parts that mix the dough, on or about October 5, 2015. The LVO SM20/SM24 moulder located in the main production room of the facility was not provided with a hopper so designed and connected to the proofer as to prevent employee contact with inrunning rolls. A guard placed on the machine was flipped into a position that did not prevent employee contact with inrunning rolls, on or about October 5, 2015.     b. The JAC moulder located in the main production room of the facility was not provided with a hopper so designed and connected to the proofer as to prevent employee contact with inrunning rolls. The hopper provided on the JAC moulder allowed employees to make contact with the inrunning rolls, on or about October 5, 2015.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
