29 CFR 1910.26(c)(3)(vii): Portable ladders were used for other uses than that for which they were intended:    On or about September 17, 2015, a portable ladder measuring 88 in height was used improperly to access the roof of a van approximately 89 in height in that three points of contact were not maintained, overreaching was necessary, and the top two steps were utilized.
