29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged:    On or about August 17th through September 4th 2015, employees were engaged in cleaning out Bins #26, #28 and #29 with the use of a sweep auger and employees were allowed to be in the bins when the sweep auger was in operation. The sweep auger was not affixed with acceptable engineering controls such as: 1. A sweep auger equipped with an attached guard that prevents the worker's contact with the unguarded portion of the auger; 2. A sweep auger equipped with a control mechanism, such as a dead-man switch or other similar device, which will allow for the sweep auger's operation only when the operator is in contact with the device. 3. The installation of guardrails or catwalks that prevent workers from entering the area within the path of the auger.
