29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about October 6, 2015, during the installation of roofing material on a low slope or flat roof at a job-site located on 20203 Lorain Road, North Olmsted, Ohio, the employer failed to ensure employees were protected against contact with an energized 110kV power line (circuit) during the course of their work, by de-energizing and grounding or effective guarding.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about October 6, 2015, during the installation of roofing material on a low slope or flat roof the employer did not ensure employees were protected from a fall of approximately 10 feet by utilizing a guardrail system, fall protection system, and/or combination of warning line system and safety monitor system.
