29 CFR 1926.651(c)(2):  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet or more in depth so as to require no more than 25 feet of lateral travel for employees:   (a)  G&C Construction, Inc. at 8500 E Mississippi Ave, Bldg 9, Denver, CO 80247:  On or before October 5, 2015 the exposing employer did not ensure that each employee conducting work in a trench that was approximately 100 feet in length, 3.5 feet in width, and 7 feet in depth was provided with a safe means of egress, in that a stairway, ladder, ramp or other safe means of egress was not located so as to require no more than 25 feet of lateral travel for employees.  Only one ladder was placed inside the trench.  This condition did not provide employees with a safe means of trench egress.

29 CFR 1926.651(j)(2):  Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into the excavation:    (a)  G&C Construction, Inc. at 8500 E Mississippi Ave, Bldg 9, Denver, CO 80247:  On or before October 5, 2015 the exposing employer did not ensure that the spoil pile was at least two feet from the edge on the west side of the trench.  The trench was approximately 100 feet in length, 3.5 feet in width, and 7 feet in depth.  This condition exposed the employees to the hazard of spoil pile material falling into the excavation or causing the excavation wall to collapse.

29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):     (a)   G&C Construction, Inc. at 8500 E Mississippi Ave, Bldg 9, Denver, CO 80247:  On or before October 5, 2015 the exposing employer did not ensure that each employee working in a trench that was approximately 100 feet in length, 3.5 feet in width, and 7 feet in depth was protected from cave-in, in that the east wall of the trench was vertical.  This condition exposed employees to a crushing hazard by excavation wall cave-in.

29 CFR 1926.651(k)(1):  Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres and or other hazardous conditions:   (a)   G&C Construction, Inc. at 8500 E Mississippi Ave, Bldg 9, Denver, CO 80247:  On or before October 5, 2015 the exposing employer did not ensure that daily inspections of excavations and the adjacent areas were made by a competent person for evidence of a situation that could have resulted in a hazardous condition, in that employees made pipe connections in a trench that was approximately 100 feet in length, 3.5 feet in width, and 7 feet in depth.  The east wall of the trench was vertical.  This condition exposed employees to a crushing hazard by excavation wall cave-in.
