The employer did not ensure that a baseline audiogram was established within 6 months of the employee's first exposure at or above the action level.      b)A temporary worker working at Berg Steel Pipe Corp., at the Pipe Plant, was exposed to continuous noise of 95.3 dBa or the equivalent noise dose of 209.9% during the 437 minute sampling period on October 8, 2015. The employer did not ensure that a baseline audiogram was established within 6 months of the employee's first exposure at or above the action level.    c) A temporary worker working at Berg Steel Pipe Corp., at the Pipe Plant, was exposed to continuous noise of 91.2 dBa or the equivalent noise dose of 118.2% during the 470 minute sampling period on October 6, 2015. The employer did not ensure that a baseline audiogram was established within 6 months of the employee's first exposure at or above the action level.    ADECCO USA INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1049318.
