29 CFR 1910.95(g)(5)(i): The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared:      The employer failed to ensure that, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram was established.      a) A temporary worker working at Berg Steel Pipe Corp., at the Pipe Plant, was exposed to continuous noise of 96.1 dBa or the equivalent noise dose of 234.6% during the 410 minute sampling period on October 7, 2015. The employer did not ensure that a baseline audiogram was established within 6 months of the employee's first exposure at or above the action level.      b)A temporary worker working at Berg Steel Pipe Corp., at the Pipe Plant, was exposed to continuous noise of 95.3 dBa or the equivalent noise dose of 209.9% during the 437 minute sampling period on October 8, 2015. The employer did not ensure that a baseline audiogram was established within 6 months of the employee's first exposure at or above the action level.    c) A temporary worker working at Berg Steel Pipe Corp., at the Pipe Plant, was exposed to continuous noise of 91.2 dBa or the equivalent noise dose of 118.2% during the 470 minute sampling period on October 6, 2015. The employer did not ensure that a baseline audiogram was established within 6 months of the employee's first exposure at or above the action level.    ADECCO USA INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1049318. Citation No. 1, Item No. 1b, and was affirmed as final order on 08/17/2015, with respect to workplace located at 330 Eden Road, Lancaster, PA 17601.    ADECCO USA INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 948156. Citation No. 1, Item No. 2, and was affirmed as final order on 01/02/2015, with respect to workplace located at One Lemon Ln NE, Atlanta, GA 30307.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:      a) On or about, September 23, 2015, throughout establishment, the employer did not ensure that temporary employees were provided with chemical-specific effective information and training on each of the hazardous chemicals in their work area.  Employees were exposed to hazardous chemicals, including, but not limited to, Greased Lighting Super Strength Cleaner & Degreaser (corrosive, carcinogen to animals, pH 12.5-13); Ammoniated Window Cleaner (corrosive, pH 9-10); Crystalline Silica; Copper Fumes; Manganese Fumes; Zinc Oxide Fumes; and Iron Oxide Fumes.    ADECCO USA INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1011405. Citation No. 1, Item No. 3, and was affirmed as final order on 06/16/2015, with respect to workplace located at 100 River Rd, Port Jervis, NE 12771.
