29 CFR 1917.5(b): The employer did not train each affected employee in a manner required by each standard under this part, which required the employer to train employees or implement a training program on the hazards covered under these standards: a. On or about October 2, 2015, at the south one dock area, an employee conducting cargo unloading operations of Motor Vessel Sunrise had not received training in the hazards associated with this task. The employee conducting this task was not made aware on how to position himself in a way that he would be clear from the load of steel plates that was suspended by a vessel crane resulting in the crushing and eventual amputation of the employee's left hand middle finger when he was struck by the load.
