On or about 5 October 2015, and at times prior thereto, a portable 30 foot fiberglass extension ladder was used to perform elevated work, in which the rubber pads on the feet were missing and the ladder was setup on a smooth concrete floor, resulting in the ladder sliding out from under the employee, causing a fall hazard of ~14 feet to the floor below.
