OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to crushing hazards while working under automotive lifts with defective or missing components.    Service Bay, 379 Russell Road, Hadley, MA 01035; Employees were required to work under and/or adjacent to extended automotive lifts with defective or missing components and that had not been inspected or maintained, exposing them to crushing hazards.  The following automotive lifts were in use with defective or missing components:  On or about October 9, 2015 and at times prior:  (a)  Rotary Lift in Bay #4 had worn cables, a cocked sheave pin, and cables were rubbing on a cylinder bracket creating a hazard.  (b) Rotary Portable Car Lift Model PAL-5E hydraulic car lift (aka the Half Lift) in Bay #5 had a non-approved modification (a bolt) to the locking leg latch handle which required employees to reach through the lift in order to lower it.  The lift frame guide on which the locking leg engages was bent and uneven, requiring employees to continually reset the locking leg into position while the lift descended.    (c) O.ME.R. s.p.a. Viva 36 hydraulic car lift (aka the Alignment Lift) in Bay #6 had safety locks bypassed and lift was leaking hydraulic oil to the point where hydraulic pressure could not be maintained.   Among other methods, one feasible and acceptable means of abatement includes, but is not limited to, following the American National Standards Institute\ Automotive Lift Institute Standard for Automotive Lifts- ANSI\ALI ALOIM-2008 (R2013) Safety Requirements for Operation, Inspection and Maintenance including but not limited to:    (a) Paragraph 4.2 Operation- Ensure operators are trained in the proper methods to inspect the lifts safely.    (b) Paragraph 4.4 Operator Responsibilities - In accordance with Section 4.4.4 ensure lift inspections are conducted daily.   (c) Paragraph 4.5 Operator Reporting - As specified by this paragraph the lift is not to be operated until the cause of the problem as been determined and appropriate repairs have been made by qualified automotive lift personnel.   (d) Paragraph 5.1 - Ensure inspections by a qualified inspector are conducted in accordance with the manufacturer's specifications.   (e) Section 6, Perform planned and repair maintenance in accordance with this section.   (f) Section 8, Damaged and/or unserviceable parts are replaced with parts approved by the original equipment manufacturer or with meeting original manufacturer specifications.

(b) Near the mechanics station the AMEREX ABC extinguisher displayed a 2012 annual inspection and the pin had been removed.
