29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  a)  On or about October 6, 2015, during drilling activities on the Delta drill press with Serial #021180W4021, located behind the welding area, the employer did not ensure that the electrical switch guard was secured to the machine.  The electrical switch guard was damaged and was being held together with tape exposing employees to electrical hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  a) On or about October 6, 2015, during drilling activities on the Delta Serial #021180W4021 drill press, located behind the welding area, the employer did not ensure that the exposed electrical flexible cord had adequate strain relief and protected the three electrical wires from abrasions where it entered the plug.  The electrical flexible cord was damaged exposing employees to electrical shock hazards.
