29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    At the time of the inspection, the employer had not implemented a hearing conservation program for production, recycling and regrind employees who had been tested and noise levels above 85 decibels n an 8 hour time weighted average were documented.

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected:    At the time of the inspection, the employer had not conducted a periodic review of the machine specific lockout procedures with each authorized employee. Equipment includes but is not limited to injection molding machines and robotics, recycling machines, and grinder.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. At the time of the inspection the following point of operation hazards were found.     a) The screw auger on #7 hopper was not equipped with a cover on the day of a hospitalization injury 9/26/2015. The injured employee was able to place his hand into the point of operation of the screw auger.     b) At the time of the inspection, two screw augers used to transport plastic resins from bunker system did not have adequate guards. A red guard and a black guard were not secured to prevent exposure to the rotating parts.      c. At the time of the inspection one of two agriculture screw augers were not equipped with guards to prevent access to the point of operation. One had been equipped with plex-i-glass barrier guards.     d. At the time of the inspection, guards on the robot on 5 on the injection molding machines did not prevent operators from physically entering the space where the robots were operating causing a struck by hazard.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  At the time of the inspection, the employer had not implemented machine specific procedures for locking out equipment such as but not limited to the new Engel injection molding machine. Employees that serviced and operated the injection molding machines were exposed to the unexpected energization and related safety hazards.    The Cabka North America was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 315979609, citation number 1, item number 1 and was affirmed as a final order on 2/6/2012, with respect to a workplace located at 105 Byassee Dr. Hazelwood, Missouri.
