29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a. Site- On or about 9/22/2015 an employee sustained an injury which amputated the tip of a finger and on 9/24/2015 an additional segment of the finger was surgically removed. The employer reported the amputation to OSHA on 10/2/2015 failing to report the amputation in a timely manner.
