29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end: (a) Jobsite: On or about October 6, 2015, an employee was working from a pic that was greater than 10 feet in length and it extended over its one support by at least three feet. This pic was not secured to prevent tipping if an employee went to work from this part of the pic.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: (a) Jobsite: On or about October 6, 2015, two employees were not protected from falling while they were working from two different ladder jack scaffolds. The fall hazards ranged from about 15 feet to about 19 feet.
