29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.   (a)   Beneath Scaffold - On or about October 2, 2015 two employees were exposed head injuries while other employees were working on a scaffold 17.2 feet above them handling stones.  None of the employees were wearing hardhats.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.   (a)   Construction Site - On or about October 2, 2015 two employees were exposed to permanent eye injuries while cutting and sanding large stones.  Two employees operated two DeWalt DW494 angle sanders without wearing eye protection.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.  (a)  Left Side of House - On or about October 2, 2015 three employees were exposed to a fall hazard and scaffold collapse due to no base-plates being installed. The employees were working 17.2 feet above the ground.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as used as a means of access.  (a)  Left Side of House - On or about October 2, 2015 three employees exposed to a fall hazard while working on a scaffold that had no ladder installed. Employees climbed the frame 17.2 feet above the ground to access the scaffold.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a)  Left Side of House - On or about October 2, 2015 three employees were exposed to a fall hazard while working on a scaffold 17.2 feet above the ground with no guardrail system or personal fall arrest system.

29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person.   (a)   Construction Site - On or about October 2, 2015 three employees were exposed to a fall hazard due to not having a experienced competent person ensuring the safe erection of scaffold. Scaffold had no guardrails installed, no ladder installed and no base-plates installed.

29 CFR 1926.454(b): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the following topics, as applicable:   (a)   Construction Site - On or about October 2, 2015 three employees was exposed to a fall hazard while working on scaffolds.  Employees were not provided training on how to erect, disassemble, move, maintain, repair and inspect scaffolds.
