29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     On or about October 16, 2015, the employer did not ensure employees were utilizing the procedures for the safe application, usage, and removal of energy controls during set up operations, requiring the placement and removal of punches and dies inside the point of operation of an Accurpress, Model; 725012, 250 ton press brake.  Energy sources include, but not limited to, electric and gravity power.
