29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the equipment grounding conductor at the point where plug(s) are attached to the receptacle(s):    a) On or about October 6, 2015, at the above located work place, employees working at and around the Robot A-Blaster were exposed to a 120 volt Portable Cooler floor fan that was missing a grounding pin. The employees were thereby exposed to an electrical hazard.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:      a) On or about October 6, 2015, at the above located workplace, the employer's OSHA 300 form for 2015 was not completed in detail as required by 1904.4 through 1904.12. In that:    i) The OSHA 300 recorded 2 cases (1. Contusion of finger on 9/2/2015; 2. Employee struck on the forearm on 9/9/2015) that did not list in Column F, the objects that injured the employees (Column F - Describe injury or illness, part of the body affected, and object/substance that directly injured or made the person ill).    ii) The OSHA 300 recorded a case (Contusion of finger on 9/2/2015) with a check in the column J (Other recordable cases) and 34 days marked in column L (On job transfer or restriction - days).

29 CFR 1910.141(c)(1)(i): The employer did not provide water closets accordance with 29 CFR 1910.141, Table J-1.   a) On or about October 6, 2015, at the above located work place, two water closets were provided for approximately 65 hourly employees.  Table J-1 required the employer to provide a minimum of 4 water closets.
