OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn hazards and smoke inhalation from the improper cleaning of exhaust stacks:      A.)  P/M National, Inc., - On or about 10/7/15, and at times prior thereto, the interior of the exhaust stacks were not cleaned.  As a result, the combustible material accumulated inside the stack creating fire hazards.

29 CFR 1910.23(a)(3): Every hatchway and chute floor opening was not guarded by one of the methods as specified in paragraphs (a)(3)(i) and (a)(3)(ii) of this section:       A.)  P/M National, Inc., ; Molding Department - On or about 10/7/15, and at times prior thereto, employees were exposed to a potential fall hazard of 6 feet into the pits below the presses.  The employees remove the cover to the pits to enter them for maintenance work.  When the cover is removed, the employees at floor level are exposed to the fall hazards due to no guardrails being installed around the opening to prevent personnel from falling into the pit opening.

A.)  P/M National Inc. - On or about 10/7/15, and at times prior thereto, the lenses in the employees prescription glasses were a standard lens and were not designed as safety lenses.
