29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:      A.)  Creative Coatings Inc., 111 Main Street, Sharpsville, PA - On or about 10/5/15, and at times prior thereto, employees were working within 5 feet of energized utility power lines while working from a tubular welded frame scaffold.  Employees were conducting work operations from the scaffold that included, but were not limited to, plastering work.       B.)  Creative Coatings Inc., 111 Main Street, Sharpsville, PA - On or about 10/5/15, and at times thereto, employees were working within 1 foot of an energized electrical drop service while working from a tubular welded frame scaffold.  Employees were conducting work operations from the scaffold that included, but were not limited to, plastering work.

A.)  Creative Coatings Inc., 111 Main Street, Sharpsville, PA - On or about 10/5/15, and at times prior thereto, employees were working within 5 feet of energized utility power lines while working from a tubular welded frame scaffold.  Employees were conducting work operations from the scaffold that included, but were not limited to, plastering work.  The standard requires a 10 foot working clearance.     B.)  Creative Coatings Inc., 111 Main Street, Sharpsville, PA - On or about 10/5/15, and at times thereto, employees were working within 1 foot of an energized electrical drop service while working from a tubular welded frame scaffold.    The standard requires a 10 foot working clearance.  Employees were conducting work operations from the scaffold that included but were not limited to, plastering work.

Creative Coatings Inc., 111 Main Street, Sharpsville, PA - On or about 10/5/15, and at times prior thereto, employees were working from a tubular welded frame scaffold that was not fully planked.  Only about half of the level the employees were working had planking.  The deck on the scaffold that the employees were working form was approximately 16 feet above ground level.  Employees conducted work operations from the scaffold that included, but was not limited to, plastering work.

Creative Coatings Inc., 111 Main Street, Sharpsville, PA - On or about 10/5/15, and at times prior thereto, employees were working on a tubular welded frame scaffold that was not inspected by a competent person.  The scaffold defects included, but were not limited to, the working level was only partially decked, some of the planks were in poor condition, the scaffold was too close to electrical lines, and the scaffolding was missing guardrails.  Employees were working from the scaffold approximately 16 feet above the ground conduction work operations that included, but were not limited to, plastering work.
