On or about October 2, 2015, and times prior to, at the MEMC Pasadena, Inc. facility in Pasadena, TX, when the employer failed to include on the Piping and Instrument Diagram valves utilized for isolation in the 92300 on page 02-20-92-24-FD-06B for the following piping :  a)  1" SLN-7424-151 / 1/2" FX-7415-154  (K-92350 Compressor) b)  4" SLN-7439-154 / 4" SLN-4829-154 / 1 1/2" SLN-7422B-154 / 1 1/2" SLN-7422A-304.2 (D-92350 1st Stage Inlet Dampener to K-92350 Compressor) c)  1 1/2" SLN-7422-304.2 / 2" FX-7417-154 (K92350 Compressor to D92351 2nd Stage Discharge Dampener) d)  2" WCS-7141-125 (To E-92353 Interstage Cooler) e)  2" FX-7419-154 (D-92352 to flare)

29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; opening process equipment or piping.  a)  On or about October 2, 2015, and times prior to, at the MEMC Pasadena, Inc. facility in Pasadena, TX, the employer did not follow the permitting procedure during the blinding process of the K92350 compressor to ensure that the employees involved had the appropriate Personal Protective Equipment (PPE) during the activity.  b)  On or about October 2, 2015, and times prior to, at the MEMC Pasadena, Inc. facility in Pasadena, TX, the employer did not ensure proper isolation for the control of hazards during the lockout/tagout and blinding process, for the K92350 compressor.
