29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:  a.  On or about 10/06/2015,  At refrigerated intermodal container/ walk-in meat freezer area:  Employees were exposed to fall hazards while working inside a walk-in freezer area in which, water or water from air has been either spilled or condensate. The water freezes forming ice over the working floors and become a slipping hazard.   Note:  Documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(d).

29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard.  a.   On or about 10/06/2015, At the Main Kitchen- between Oven/dishwashing area:  Employees were working over an uneven surfaces with broken and loose tiles in which they could accidentally walk on, trip and fall.  Note:  Documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(d).

29 CFR 1910.23(a)(9): Every floor hole into which persons can accidentally walk (on account of fixed machinery, equipment, or walls) was not protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.  a.    On or about 10/06/2015, At the kitchen/kettle area:  Employees worked and walked by a  three (3) square floor holes  of 9"X9" that were not protected with a covers exposing them to trips and fall hazards.  Note:  Certification of abatement must submitted for these citation items as required by 29 CFR 1903.19(c).

29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  On or about  10/06/2015,  At refrigerated intermodal container/ walk-in meat freezer area:  The freezer door does not have a door safety latch mechanism or other means to open, thus in case that the freezer door was inadvertently closed employees traveling in and out from the walk-in freezer are exposed to being locked inside.  Note:  Documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(d).
