29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a) Anchor's Cove, Charleston: Employees were applying shingles to the roofs of a series of 4 connected townhomes with roof pitch of up to 12:12 and were not protected from falls that measured as high as approximately 23.5 feet from eave to ground, as observed on October 7, 2015 and times prior.     ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) Anchor's Cove, Charleston: The employer did not instruct employees on unsafe working conditions as evident by the safety hazards found on site including, but not limited to, employees not utilizing a fall protection system and employees not wearing eye protection, as determined on or about October 7, 2015 and times prior.   ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM.  Let Me Do It Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.21(b)(2) which was contained in OSHA inspection number 909227, citation number 1, item number 3b and was affirmed as a final order on 8/28/13, with respect to a workplace located at The Ridges, Lot 50 South Charleston, WV 25309.
