On or about October 8, 2015, during the installation of roofing materials on a residential home located at 3643 Rosemont, Fairlawn, Ohio 44333, the employer did not ensure employees were protected from potential eye injuries while using an air powered nail gun. The Ivan's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102(a)(1), which was contained in OSHA inspection number 745301 citation number 1 item number 3 and was affirmed as a final order on 9/4/2013, with respect to a workplace located in Canton Ohio.

The Ivan's Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1925.501(b)(1), which was contained in OSHA inspection number 745301, citation number 1, item number 4 and was affirmed as a final order on 9/4/2013, with respect to a workplace located in Canton Ohio.
