On or about October 7, 2015, roof area, employees installing a TPO roof system was not protected from falling 27 feet 6-inches by the use of a guardrail system, safety net system, personal fall arrest system or by any other type of fall protection method.

On or about October 7, 2015, employees used hazardous chemicals such as but not limited to bonding adhesive, glues, membrane cleaner and universal caulking while working. The employer did not provide an explanation of the new labeling system, safety data sheets and pictograms.
