29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a)  225 Washington Street, Conshohocken, PA, Worksite - On or about October 6, 2015, employees are working on a low pitch roof, no less than 40 feet above grade, without utilizing any means of fall protection.    DDP Contracting, Inc. dba DDP Roofing Services Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard, which was contained in OSHA inspection number 942316, citation number 1, item 1, which was affirmed a final order on 2/3/14, with respect to a workplace located at 255 Northland Center, State College, PA, contained in OSHA Inspection No. 459035, citation number 1, item 1 and was affirmed a final order on 7/16/12, with respect to a workplace located at 272 N. Lake St., Aurora, IL, and contained in OSHA inspection number 314724402, citation number 1, item number 2 and was affirmed as a final order on 9/14/11, with respect to a workplace located at 2590 MacArthur Rd., Whitehall, PA.
