29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. (a) Worksite was located at 17 Cable Drive, Kearny, NJ 07032. Employees were observed using Crown SP Series Type E Stockpickers with fork adapters on them that the machine did not list as an approved attachment on the equipment data plate. The trucks bore no other identification tag that listed the attachment. Violation was observed on or about 10/7/2015.

(a) Workplace was located at 17 Cable Drive, Kearny, NJ 07032. Employees were exposed to an Ametek Battery Mate 80 Charging Station, Serial Number 413CS14354 that had its power cable pulled out at the strain relief giving electrical wires no abrasion protection from the metal frame of the charger. Employees were also exposed to charging cables that were badly chaffed exposing live electrical conductors to a steel rack system. Violation was observed on or about 10/7/2015. (b) Workplace was located at 17 Cable Drive, Kearny, NJ 07032. Employees were exposed to a Northeast Battery Charger Station, Model Number 3NE24-750, Serial Number 10357G2, that had bare wire exposed at the base of the charging cable. Employees working with this station were exposed to live electrical contacts. Violation was observed on or about 10/7/2015.

Violation was observed on or about 11/12/2015.
