29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee (s) was required to (A) Remove or bypass a guard or other safety device; (B) were required to place any part of his or her body into area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle:   a) On or about 10/09/2015, in the shop area when employees service machines, such as changing the blade on the horizontal band saw, energy control procedures were not being implemented. 29 CFR 1910.147(c)(6)(ii): The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment, inspection date, employees included, and the person performing the energy control procedure.  a) On or about 10/09/2015, in the machine shop, employer fail to appropriately identify and document the machine or equipment on which the energy control procedure was being utilized, such as but not limited to horizontal band saw.   ABATEMENT CERTIFICATION REQUIRED
