29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Job site, 311 Siebert Road, Pittsburgh, PA: On or about October 6, 2015, during excavation activities, the employer did not ensure that appropriate cave in protection was in use where work was being performed in an excavation determined to be at least approximately 5 foot 1 inch to 5 foot 4 inches in depth.
