29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems: At the construction site, employees installing shingles on a steep roof without fall protection were exposed to falling approximately 18 feet to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the construction site, employees used ladders that were not extended at least 3 feet above the roof line to access the working area. Employees were exposed to falling more than 18-feet.
