29 CFR 1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):        Professional Remodelings Inc. at 501 Lipan St, CO 80223:  On and before September 18, 2015, the exposing employer did not ensure a safety program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person. Four subcontractor employees were working next to the edge of the roof on a one-story residential home without the use of a fall protection system. The subcontractor employees were conducting roofing activities. This condition exposed employees to a fall hazards of approximately 10-15 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Professional Remodelings Inc. at 1942 S. Lincoln St., Denver, CO 80210: On or before September 18, 2015, the exposing employer did not ensure the use of fall protection when roofing this single-story family home. The condition exposed employees to fall hazards up to 15 feet.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a)  Professional Remodelings Inc. At at 1942 S. Lincoln St., Denver, CO 80210: On or before September 18, 2015, the exposing employer did not ensure a written fall protection training certification record was prepared which included the identities of employees trained, dates of training, and the signature of the trainer or employer. Three employees engaged in roofing activities on a single story residential home were exposed to fall hazards up to 15 feet without fall protection.
