29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:    a)       Tire Storage Loft;  There was a 37-39 inch high guard rail with no mid-rail, exposing employees to potential fall hazards.

29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:     a)  Tire Storage Area;  The exit discharge door was padlocked the morning of October 7, 2015, then the lock was removed during the walkthrough. There was also a metal bar running the length of the door, which was attached to the door and must be lifted up and removed to open and exit through the doors. This metal bar is in place 24 hours a day 7 days a week, unless the door is opened during deliveries. This exposes employees to delayed egress hazards in case of emergency.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a)   A Frigidaire refrigerator was plugged into an office rated surge protector (relocatable power tap). The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.    (b)  Oster microwave and Coffee Maker were plugged into an office rated surge protector (relocatable power tap). The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.    (c)  An office rated surge protector (relocatable power tap) was used in the auto bays to charge Snap On Tools battery chargers for drills. The UL listing states that office rated relocatable power taps are not designed for dirty, greasy auto shop environments.  This exposes employees to possible electrical fire hazards.
