29 CFR 1910.213(h)(3): Radial saw(s) were not provided with an adjustable stop to prevent the forward travel of the blade beyond the position necessary to complete the cut in repetitive operations: (a) Dock Assembly Area: Employees operate a Rockwell 12 inch radial arm saw (serial #EV5425) to cut dimensional lumber for boat docks. The bottom portion of the blade of the radial arm saw extended up to 5 inches past the cutting table in forward traveling position, as determined on October 7, 2015.

29 CFR 1910.219(e)(2)(i): Overhead horizontal belt(s) with lower parts seven -7 feet or less from the floor(s) or platform(s) were not guarded on the sides and bottom in accordance with the requirements specified at CFR 1910.219(o)(3): (a) Fabrication Area: The employer did not provide guarding for the mechanical power transmission apparatus on a Birmingham milling machine (serial #991317, model #BPS-1642). A 3 inch by 11 inch opening was not guarded exposing employees to a horizontal moving 1/2 inch V-belt, as determined on October 7, 2015.
