The bottom portion of the blade of the radial arm saw extended up to 5 inches past the cutting table in forward traveling position, as determined on October 7, 2015.

A 3 inch by 11 inch opening was not guarded exposing employees to a horizontal moving 1/2 inch V-belt, as determined on October 7, 2015.
