29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   (a) Regis Gnl Solutions, LLC at 3400 S Lowell Blvd., Denver, CO 80236:  On and before October 6th, 2015 the employer did not ensure that frequent and regular inspections by a competent person to identify work site hazards.  Employees performing roofing operations were not protected from fall hazards and projectile hazards as the following conditions were identified onsite:     (1) Employees performing roofing operations on two third story apartment buildings were exposed to fall hazards and the employer did not ensure that employees were protected from fall hazards through the use of fall protection.  This condition exposed employees to fall hazards in excess of 30 feet.  (2) An employee performing pneumatic nailing operations without eye protection was not protected from flying debris and the employer did not provide and require the use of eye protection.  This condition exposed the employee to projectile hazards to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Regis Gnl Solutions, LLC at 3400 S Lowell Blvd., Denver, CO 80236:  On and before October 6th, 2015 the employer did not ensure that employees were protected from falling off sloped roofs during roofing work.  Employees were performing roofing work on buildings 2 and 13 without the use of fall protection nor had the employer implemented an adequate fall protection plan.  This condition exposed employees to fall hazards in excess of 30 feet.

(a) Regis Gnl Solutions, LLC at 3400 S Lowell Blvd., Denver, CO 80236:  On and before October 6th, 2015 the employer did not ensure that an employee using a pneumatic nailer capable of producing eye injury was protected from flying debris.  This condition exposed the employee to projectile hazards to the eyes.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  (a) Regis Gnl Solutions, LLC at 3400 S Lowell Blvd., Denver, CO 80236:  On and before October 6th, 2015the employer did not ensure that the secured portable extension ladders which did not extend three feet above the landing surface were outfitted with grasping devices to assist employees in mounting and dismounting the ladders.  This condition exposed employees to fall hazards in excess of 30 feet.
