29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about October 07, 2015, employee was exposed to falls greater than 6 feet in height to the ground level below, when not wearing a safety harness with lanyard attached to the JLG Model 400S aerial lift with serial #0300119488, while riding the lift to access the roof.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about October 07, 2015, employee with no fall protection was exposed to falls greater than 6 feet in height to the ground level below, when working at the very edge of the roof, while spray painting the welds.
