29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):     (a) SRG Roofing LLC at 3400 S Lowell Blvd., Denver, CO 80236:  On and before October 6th, 2015 the employer did not ensure that frequent and regular inspections by a competent person to identify work site hazards.  Employees were performing roofing operations without fall protection on two three story buildings.  This condition exposed employees to fall hazards in excess of 30 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) SRG Roofing LLC at 3400 S Lowell Blvd., Denver, CO 80236:  On and before October 6th, 2015 the employer did not ensure that employees were protected from falling off sloped roofs during roofing work.  Employees were performing roofing work on buildings 2 and 13 without the use of fall protection nor had the employer implemented an adequate fall protection plan.  This condition exposed employees to fall hazards in excess of 30 feet.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    (a) SRG Roofing LLC at 3400 S Lowell Blvd., Denver, CO 80236:  On and before October 6th, 2015 the employer did not ensure that the secured portable extension ladders which did not extend three feet above the landing surface were outfitted with grasping devices to assist employees in mounting and dismounting the ladders.  This condition exposed employees to fall hazards in excess of 30 feet.
