29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter: a) In the plant, on or about October 1, 2015 - Employees used tight-fitting air purifying respirators such as Moldex 2300 N95 respirator. Employee did not participate in annual fit test. b) In the plant, on or about October 1, 2015 - Employees used tight-fitting full face respirators such as a Sperian Survivor respirator. Employee did not participate in fit test with the full face respirator or on an annual basis.
