29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: (a) East side, commercial construction located at 723 Hwy 138, Riverdale, GA 30274: The employer allowed employees to work from a 4-ft. wide x 24-ft. long makeshift scaffold platform which was resting on the forks of a SkyTrak 5030 Forklift Truck. The platform attachment was not approved or designed by the manufacturer to be used with this equipment. An employee was working from this platform at heights of approximately 14-feet, 3-inches from the lower level.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) East side, commercial construction located at 723 Hwy 138, Riverdale, GA 30274: Employee was engaged in stucco application and exposed to falls hazards of approximately 14-feet, 3-inches to the lower level while working from a makeshift scaffold platform which was resting on the forks of a SkyTrak (Model 5030) Forklift Truck and not secured to prevent accidental movement during use.

(a) East side, commercial construction located at 723 Hwy 138, Riverdale, GA 30274: Employee was exposed to falls while working from an elevated work platform at heights of approximately 14-feet, 3-inches with no fall protection system in place. The employee was working from a makeshift scaffold platform without guardrails or a personal fall arrest system.
