29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On a commercial building, on or about October 7, 2015, employees were exposed to struck-by hazards to the head while cutting lumber from ground level near employees working from the first level of a scaffold and not wearing protective helmets.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces and side brackets were used as a means of access:     On or about October 7, 2015, employee(s) were exposed to falls greater than 9.5 feet in height to the ground level below from the work platform of the scaffold, which did not have a ladder for access and egress to and from the scaffold platforms, while using the scaffold to access the rooftop.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about October 7, 2015, employees were exposed to fall hazards greater than 6 feet in height to unpaved ground level below while installing tyvek material on an exterior wall from the unprotected edge of a 1st level rooftop without wearing fall protection nor were there any other measures of fall protection in place.

29 CFR 1926.503(c): The employer did not retrain affected employees  who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:  On the roof of a commercial building, North side, on or about October 7, 2015, employees were exposed to a fall hazard greater than 9.5 feet to the unpaved ground below, while not being trained on the procedures to recognize fall hazards.
