On or about October 7, 2015 on the first floor main electrical room at 4565 Ashford Dunwoody Road, Dunwoody, Ga. employees working in the area were exposed to a tripping hazard when the cover over the hole was not secured over a hole fifteen inches deep and 27 inches wide.
