29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. United States Postal Service; 1801 Brooks Rd. Richmond, VA 23232: On 10-01-2015 at about 1:45 P.M. , a USPS Mail Carrier suffered a broken ankle after a fall resulting in emergency surgery and formal in-patient hospitalization admission to the hospital. The employer did not report the in-patient hospitalization to the Occupational Safety & Health Administration until 10-05-2015 at 8:15 A.M. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
