OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from potential fire and explosion hazards:    (a) Facility, 175 Wade Street, Waynesburg, PA:  On or about October 13, 2015 employees were exposed to potential burn and fire hazards from the Chicago GA-18 Flatwork Ironer due to improper maintenance.  The ironer did not maintain the required gas pressure to ensure the equipment operated properly.  The burner control would trip when the gas pressure was too low and required resetting by the employees.  The burner control would trip continually and the equipment would not maintain the required heat needed to press materials.  Occasionally the gas will build up and when the igniter would light a burst of flame would occur exposing employees to burn and fire hazards.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    (a) Facility, 175 Wade Street, Waynesburg, PA:  On or about October 13, 2015 the electrical control equipment on the Chicago ironer operating at 208v was not guarded or covered to protect employees from accidental contact with the energized parts.  The employees reach into the control equipment to reset the burner controller and are exposed to potential electrical shock hazards.

(b) Facility, 175 Wade Street, Waynesburg, PA:  On or about October 13, 2015 no cover was in place on the junction box for the washers.      29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:    (a) Facility, 175 Wade Street, Waynesburg, PA:  On or about October 13, 2015 an outlet box located on the wall near the smaller iron did not have a cover in place.  The outlet was being used to supply power to two flexible cords.
