OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from potential fire and explosion hazards: (a) Facility, 175 Wade Street, Waynesburg, PA: On or about October 13, 2015 employees were exposed to potential burn and fire hazards from the Chicago GA-18 Flatwork Ironer due to improper maintenance. The ironer did not maintain the required gas pressure to ensure the equipment operated properly. The burner control would trip when the gas pressure was too low and required resetting by the employees. The burner control would trip continually and the equipment would not maintain the required heat needed to press materials. Occasionally the gas will build up and when the igniter would light a burst of flame would occur exposing employees to burn and fire hazards.

The employees reach into the control equipment to reset the burner controller and are exposed to potential electrical shock hazards.
