29 CFR 1926.1402(b): The equipment was assembled or used without ensuring that ground conditions are firm, drained, and graded to a sufficient extent so that, in conjunction (if necessary) with the use of supporting materials, the equipment manufacturer29 CFR 1926.1402(b): On or about October 8, 2015 at 3218 Tobie Circle, Scottsdale, Georgia, the boom truck used to lift drywall material to the third floor, outriggers were not supported in such a manner to distribute the weight of the boom truck and drywall material, when the operator set up the outrigger on a wet, muddy surface without pads.

29 CFR 1926.1417(e)(1)(iv): Barricades or caution lines, and notices, were not erected to prevent all employees from entering the fall zone. No employees, including those listed in 1926.1425(b)(1) through (3), 1926.1425(d) or 1926.1425(e), are permitted in the fall zone. On or about October 8, 2015 at 3218 Tobie Circle, Scottsdale, Georgia, the boom truck used to lift drywall material to the third floor, the area around the boom truck was not barricaded or blocked off so that other employees could venture into the free fall area.
