29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a)  Worksite, 1625-1629 Biltmore Lane, Pittsburgh PA, On or about October 7, 2015 - The ground employees were cutting the smooth surface trim and wood which generated aluminum and wood dust without wearing any eye protection.

The program shall enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  Worksite, 1625-1629 Biltmore Lane, Pittsburgh PA, On or about October 7, 2015 - Employees were working unprotected and exposed to fall hazards of between approximately 12 to 15 feet while installing siding to a new construction townhome community.  The employee was not trained to recognize the hazards of working at heights and how to protect against them.

29 CFR 1926.1053(b)(7): Ladders were used on slippery surfaces without being secured or provided with slip-resistant feet to prevent accidental displacement:   a)  Worksite, 1625-1629 Biltmore Lane, Pittsburgh PA, On or about October 7, 2015 - A ladder was placed on uneven ground causing the footing to be unstable. This condition exposed the employee on the 8ft ladder to a fall hazard.
