29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): a) Work site - On or about October 13, 2015 and at times prior thereto, employees were installing a water line in a trench with vertical walls and had no protective system in place. The trench measured between 7' and 8'4" deep, 6' wide, and it was approximately 20' in length.
