29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts: a. On or about 10/20/2105, at the RWC line, employees walking near and working at the reject station/ drop off point of the RWC are exposed to amputation and caught-in hazards where the conveyor system is not guarded against an employees hands being placed into the line. b. On or about 10/20/2015, at the RWC line, employees are exposed to caught-in hazards while loading the tubes into the fixtures, the employees physically hold the tubes in place until the clamps close. Employees' gloves are periodically caught in the fixtures and their fingers become pinched. c. On or about 9/23/2105, at the RWC loading station, an employee suffered an amputation of his left index finger to the first joint while reaching into the conveyor to retrieve a rejected part that was not properly ejected prior to reaching his position. There was no guarding in place to prevent his hands from entering into the hazard areas.
