29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with the General safety and health provisions covered in 29 CFR 1926.20.  a)  On or about October 13, 2015, during roofing work, at a site located at 723 Stark Street in Wausau, WI 54403, the employer had not developed and implemented a safety and health program that provided for frequent and regular inspections of the job site, materials, and equipment.  The employer also did not develop and implement a safety and health program to ensure that employees recognized and the proper procedures were followed for fall hazards greater than 6 feet and the proper use of ladders on construction sites.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On or about October 13, 2015, during roofing work at a site located at 723 Stark Street in Wausau, WI 54403, the employer failed to ensure that employees use a fall protection system when exposed to falls greater than 16 feet to the ground on a high slope roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  On and or about October 13, 2015, during roofing work, 1586 Schaefer Circle, Appleton, WI 54915, the employer did not ensure that each employee engaged in roofing work had been trained in the fall protection requirements to identify and implement conventional or personal fall protection while working on surfaces from approximately 16 feet and greater from the lower ground level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a)  On and or about October 13, 2015, during roofing work at 723 Stark Street in Wausau, WI 54403, the employer did not ensure that the silver portable KPro Non-Conductive Fiberglass ladder leaning against the left hand of the house was extended 3 feet to access upper landing surfaces and was not secured at the top of the ladder to prevent displacement.
