29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. a. On or about October 14, 2015, employees were required to periodically work in the ammonia room. The exit door in the ammonia room did not open freely as designed from the inside as required.

29 CFR 1910.146(c)(1): The employer shall evaluate the workplace to determine if any spaces are permit-required confined spaces. NOTE: Proper application of the decision flow chart in Appendix A to section 1910.146 would facilitate compliance with this requirement. a. On or about October 14, 2015, in the plant, employees are required to enter attic space above the ovens to repair water lines. The employer did not evaluate the attic space to determine if it was a permit required confined space.
