(a) On or about October 20, 2015 at worksite 5720 Brighton Ave, Kansas City, Missouri.-employees in the machine shop area were exposed  to a caught in or struck by hazards from a dual bench grinder, which was missing the right side work rest and the left side was not properly adjusted within the required one-eighth inch.
