29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Propane Refueling Area - On or about October 13, 2015, employees installing 33-pound propane cylinders, as a means of refueling forklifts, were not wearing the necessary personal protective equipment to prevent injury in the event of an inadvertent release of compressed propane fuel.
